Client Right #6 states: "A right to social interaction and participation in community activities."
SIR stands for Special Incident Report. As a long-term care facility we need to report certain incidents to San Diego Regional Center (SDRC) by filling out the SIR form and submitting it within 48 hours of the occurrence. An example of a special incident that needs to be reported to SDRC would be a serious injury/accident such as a fracture or laceration requiring sutures or staples. When we make a report to SDRC, we must also submit an Unusual Incident Report (UII) to Community Care Licensing. Please refer to the Dropbox training folder for additional reading on SIR Training.

Firstly, the type of special incident reports that must be documented and reported to the SDRC are SIRs—special incident reports. These are any events or instances that involve AWOLs, missing persons, hospitalization and abuse. This must be reported because it involves incidents or events that impact or detract from a client’s health, safety, life, and rights. Anything of this nature is to be included into what is sent to the SDRC.
Secondly, in SIRs, certain data must be included or entered on them. Such that pertains includes a date, time, place and names. It also requires a summary of the incident, the response taken by staff and indication of hospitals/police were contacted. This is all confidential, private and most importantly,…
1.-AWOL or missing, hospitalizations, any type of abuse/neglet.
2.- SIR should include name,DOB, type of incident, time and date
1) Abuse, neglect, hospital visits, missing persons
2) A special incident repost should include name, birthday, and they type of incident with date and time
If there is a incident of abuse,injury need to call 911 and get all information and do the reports SIR UII with client name, time location and what happen.
What needs to be reported to SDRC
Abuse or neglect, AWOL, medical treatment and any hospitalizations.
For the SIR managers, 911 or any law enforcement needs to be notified, has to have detailed description of incident, date and time, location, any witnesses and action that was taken.
