When to Document a Special Incident Report (SIR)

Reporting incidents that occur within the homes is very important. A special incident report (SIR) and a unusual incident report (UII) are documents that are written and reported to San Diego Regional Center and Licensing as required depending on the situation. How do we determine if an incident is reportable or not? One example, would be to write a SIR if a client is missing or goes AWOL from the facility and you have filed a missing person report with a law enforcement agency. Let's discuss the key factors that would help you identify if you must write a SIR/UII report.
