SIR stands for Special Incident Report. As a long-term care facility we need to report certain incidents to San Diego Regional Center (SDRC) by filling out the SIR form and submitting it within 48 hours of the occurrence. An example of a special incident that needs to be reported to SDRC would be a serious injury/accident such as a fracture or laceration requiring sutures or staples. When we make a report to SDRC, we must also submit an unusual incident report (UII) to Community Care Licensing. Please refer to the Dropbox training folder for additional reading on SIR Training.
1) What special incidents need to be reported to SDRC?
2) What information needs to be included in the special incident report?

SDRC report needs to be done when the police or emergency are call for example:a client is in need of hospital,the date it happen,time,names of police or EMT were are they are taking them.
1.Abuse within reasonable suspicion or proof, neglect, self harm, an AWOL or even death
2. information that needs to be on the SIR is Personal information such as Name , birthdate time, place, & date of the incident. Explanation in detail of what occured and if there were other parties involved with any info on them as well as all emergency contact information.
Abuse, neglect, serious injury, assault of any kind, awol and death.
All of the residents information, time and date of the incident, when it is reported to SDRC
As well as a detailed description of the incident with an event # and if any medical treatment was needed.
1. Reasonably suspected abuse,neglected, serious injury, unplanned hospitalization, death, etc. 2. Client information, location/time of incident, description of the incident, information about any perpetrator, name and contact info of all parties.
What special incidents need to be reported to SDRC? 1) Serious Injury, All Forms Of Abuse , Death,Suicide Attempts . What information needs to be included in the special incident report ? 2)The Date and time of incident , location, description of damages , Name and contact info of all parties involved .
