Determining when to write/report a SIR?

Reporting incidents that occur within the homes is very important. A special incident report (SIR) and a unusual incident report (UII) are documents that are written and reported to San Diego Regional Center and Licensing as required depending on the situation. How do we determine if an incident is reportable or not? Let's discuss the key factors that would help you identify if you must write a SIR/UII report.

A Special Incident Report, or SIR, is a formal report that is filed with the regional center whenever a person served by the regional center is involved in an unusual event, such as a crime or injury. There are detailed and specific rules about SIRs that service providers and regional centers must follow whenever a special incident occurs.

An SIR/UII is required when: - Client AWOLs - Suspected abuse - Serious injury or accident Important items to have included in an SIR/UII include: who, what, when and where the incident occurred. Names of witnesses are also required, as well as incident/case number from police (if contacted). The proper way to save a SIR/UII report is: Date_clientinitials_SIR or UII (ex. 07_02_20_JD_SIR or 07_02_20_JD_UII) You must also place it in the clients individual SIR/UII folder on Dropbox. Make sure to use ‘save as’ and not just ‘save’ when completing your document. Otherwise you will save over the template.

1. A sir/uii would be required if there is a severe behavior that results in and injury or damage of house/personal property as well as any type of sexual harassment or awol.


2. Some important items that would need to be in the written body would be the exact time of the incident, thorough description of exactly what happened and who was included in that incident. Whether or not law enforcement or medical attention was needed and if so, get ALL information from the police, fire department or EMT including name badge/identification number and where the resident would be taken if they were to be removed from the facility. When is a SIR/UII required?

Answer: In some circumstances, at anytime, a SIR/UII is required, where a behavior turns into a accident needs to be documented, or emergencies 911, police involvement, physical contact, putting staff, or other clients at risk, sexual harassment, abuse etc.

Answer: When the accident occurred, where a place, accuracy of position, where about’s and why, what was the purpose of etc.
