Documenting a Special Incident Report (SIR)

All long-term care facilities shall report to the regional center any Special Incidents if they occurred during the time the consumer (Client) was receiving services and supports from a long term care facility. One example, would be to write a Special Incident Report if a client is missing or goes AWOL from the facility and you have filed a missing person report with a law enforcement agency.


1) Give two other examples of when a Special Incident Report (SIR) needs to be written. Also, when writing an SIR what are some details that should be included in the body of the paragraph to make it a complete SIR?

One example of and incident that would require an SIR would be of there was an assault on someone in the facility, especially if it is a resident.

Another example would be if a resident was having a medical issue that required immediate emergency medical attention.

When writing an SIR you always want to include and event number, the name of a law enforcement or medical personnel, as well as a badge number.

When a special incident report SIR need to be written. When Injury accidents- Dislocation, Fractures, Missing person. The details that should be included in the body of the paragraph to make it a complete SIR is the SIR following up Documentation Action who, what, when, where, why and how to make a complete report.

2 some example of SIR needs to be written : property damage , and a fall Every incident report you file shld contain a minimum of the following: > type of incident( injury, property damage, theft, awol ) > address > date of incident > time of incident >name of affected individual >a narrative description of the incident including the sequence of events and results of the incident > injuries if any >treatment required if any >witness name >witness statements >other workers involved >video and photographs of the scene

Also, when writing an SIR what are some details that should be included in the body of the paragraph to make it a complete SIR?


  • A full detailed report of what happened (who, what, when, where)

  • Make sure to include the officers information
